Summary:
As a member of the PACE Interdisciplinary Team, the Registered Nurse participates in the planning, implementation, and evaluation of care plans that meet the objectives, standards, and policies of the PACE model of care. The Registered Nurse will assess the patients’ needs for home care services while planning, scheduling and performing visits. Hours are 8a-4p, Monday through Friday.

Responsibilities:

• Complete initial and periodic assessments in patients’ homes; develop and revise care plans
• Conduct visits to all patients to reassess frequency and services needed
• Review LTC patients charts monthly, performing annual assessments at minimum.
• Record and maintain accurate documentation, while completing proper authorization forms.
• Provide education and consultation to participants and caregivers on care plans, treatments, and medications
• Participate in after-hour visits as needed
• Frequent local travel

Requirements:

• Current RN license in Massachusetts, or ability to obtain before start date
• Current CPR Certification
• Covid Vaccination and fit test required
• 2 years of Home Care experience preferred
