Requirements of the Hospice Case Manager:
- Experience with hospice case management required
- Experience providing hospice care in a home setting required.
- Prior experience with rural travel preferred.
- Experience with McKesson/Netsmart EMR preferred
- Work schedule Monday – Friday
- 2+ years of hospice case management experience required
- VT/Compact RN license required
- BLS required
