Location: North Lynn, MA

We are seeking a compassionate and proactive Senior Care Nurse Practitioner to join a respected outpatient and home health program serving aging adults in North Lynn, MA. This full-time, in-person position plays a key role in identifying and addressing the complex health needs of seniors. Frequent local travel is required, with weekday daytime hours offering a consistent schedule and no weekend work.

Responsibilities of the Senior Care Nurse Practitioner:

• Conduct thorough assessments to evaluate the aging process and incorporate findings into individualized care plans.

• Collaborate closely with an interdisciplinary care team to support seniors’ physical, emotional, and social health needs.

• Assist patients in navigating end-of-life care decisions and completing advanced directives.

• Gather detailed information from patients and families to understand home environments and care challenges.

• Ensure care plans reflect a holistic view of each patient’s condition and lifestyle.

Requirements of the Senior Care Nurse Practitioner:

• Current state licensure as a Nurse Practitioner.

• Elder care experience preferred.

• Current CPR Certification.

• COVID-19 and Flu vaccinations required.

• Access to a personal vehicle for frequent local travel.
